Purpose and Reason:

The DCYF Case Plan/Agreement, in compliance with Federal and State law, is time limited and goal oriented and identifies the needs and proposed services for the parent(s) and child(ren). It delineates the mutual responsibilities and expectations of the parent(s), the child(ren) and the Department towards reaching the identified service goal. It is a tool designed to enhance service delivery.
